Every pupil entering the Morris School District schools for the first time shall present a certificate from a
licensed physician attesting to his/her physical condition. Parents/guardians who because of religious
convictions do not have their children examined must state this in writing.
Health Examinations
Every pupil shall be examined upon entry to school. The physical examination shall be conducted at the
medical home of the pupil, and a full report must be sent to the school. If the pupil does not have a
medical home, the district will provide the examination at the school physician’s office or other
appropriately equipped office. The examination will be documented on a form in accordance with
recommendations by the Commissioner of Education.
The Morris School District administrator responsible for nursing services, or his/her designee, will notify
parents of the importance of obtaining subsequent medical examinations of the pupil at least one time
during each developmental stage at early childhood (grades preschool through grade three), pre-adolescence
(grades four through six), and adolescence (grades seven through twelve).
Pupil health examinations shall include, in accordance with district regulations; immunizations; the pupil’s
health history; height, weight, hearing, and vision screenings; and physical examination of the pupil’s
body. Pupil examinations/physicals and pupil athletic physicals hall not require screening or disclosure of
HIV status.
Each candidate for a place on a school athletic squad or team shall submit to a medical examination
conducted in accordance with Policy 5141.3 and rules of the State Board of Education.
Pupils referred for evaluation for eligibility for special education and/or related services shall be examined
in accordance with Policies 6171.4 and 6164.4 rules of the State Board of Education.
Pupils suspected of being under the influence of alcohol, drugs, or anabolic steroids shall be examined in
accordance with Policy 5131.6 and rules of the State Board of Education.
Scoliosis Screening
Each pupil between the ages of ten and eighteen years shall be examined annually for scoliosis by a
school employee trained in scoliosis screening. A pupil shall be exempt from the examination on the
written request of the parent(s) or legal guardian(s).
Every pupil entering the Morris School District for the first time shall present a record of immunization as
required by law. Exemptions for religious reasons are permitted upon certification by parents/guardians
that immunization interferes with the free exercise of a pupil’s rights.
Children who have not completed immunizations may be admitted to school provided the necessary
immunizations are being received in the fastest time consistent with the approved immunization schedule
and good medical practice.
Immunization is primarily the responsibility of the parents/guardians. Any immunization program
conducted by this school district requires prior approval of the board of education and can only extend to
those immunizations provided for in the state sanitary code.
The Superintendent or his/her designee shall formulate regulations for athletic participation physical
examinations. He/she, in cooperation with the medical inspector, shall ensure that all specific
examinations required by statutes or code are carried out according to law. Any health defects revealed
by an examination given by the school health services must be reported to the parent/guardian.
